INSTRUCTIONS
This schedule provides detail in support of the inventory amount(s) for each position holder by product type. Upon prior approval, you may submit a
schedule summarizing your inventory schedules, which must be formatted the same as the West Virginia Terminal Operator’s Schedule of Inventories
(WV/GAS-503C). If you choose to submit summary schedules, you must still submit the schedules of individual inventories.
Complete Separate Schedules for Each Product Type and Group Position Holders Together

Use Net Gallons for this report.
FEIN/SSN
Enter the Federal Employment Identification Number (FEIN) or
Social Security Number (SSN) as shown on your monthly West Virginia Terminal Operator’s Report.
Name
Enter the Name of the Company as shown on your monthly West Virginia Terminal Operator’s Report.
Terminal Control Number
Enter the Terminal Control Number assigned by the IRS.
Report Month/Year
Enter the Month and Year you are reporting.
Product Information
Check the applicable box for the product type accounted for on this schedule.
Position Holder’s Name
Enter the Name of the Company that owns the product as shown on your terminal records.
Position Holder’s FEIN/SSN
Enter the FEIN or SSN of the position holder that owns the product as shown on your terminal records.
Beginning Inventory
Enter the number of Net Gallons in the terminal at the beginning of the reporting period.
Note – These figures must match the Ending Inventory from the previous month’s report.
Total Receipts
Enter the number of Net Gallons received into the terminal during the period.
Total Disbursements
Enter the Net Gallons disbursed from the terminal during the period.
Gain/Loss
Enter the Net Gallons of product gain or loss during the period.
Actual Ending Inventory
Enter the result of the following:
Gain – Column 5 plus Column 6.
Loss – Column 5 minus Column 6
