Q. How do I file a complaint with the Department?
You may file a complaint by telephone, mail, online or fax. By telephone, you may call the Department’s
Central Complaint Registry, 8:30 a.m. - 4:30 p.m., Monday-Friday at 800-252-4343. You also may submit
your complaint in writing to:

Yes. Below is a list of other state agencies.

Q. Who should I contact to check the status of my complaint?
To check the status of your long-term care complaint, contact the Department’s Bureau of Long-Term
Care at 800-252-4343. For non-long term care complaints contact the Department’s Division of
Health Care Facilities and Programs at 217-782-7412. To make inquiries, you must have the name and
location of the facility. This is NOT a toll-free call.
