Personalized Campaigns
Year after year, results have indicated that personalization has significant impact on employee donations
and overall campaign growth. It is a great way to engage donors and increase your campaign.
To arrange personalized pledge forms for the upcoming campaign, complete the attached request form and
submit it to United Way, along with a current electronic employee file in Excel or Comma Delimited (CSV).
United Way requires a minimum of 15 working days to produce your personalized campaign.

Helpful hints to assist you filling out this form
Employee information printed on pledge form:
These are fields that would be printed on the form. Three fields are mandatory. First name, Last name, and
company name. Other common fields are department, employee number, branch, location. You can select if
you would like the information printed in the window of the envelope.
The sorting of your pledge packages:
You can indicate how you would like your packages sorted. For example:
First sort:
First Name
Second sort:
Last Name
Third sort:
Department
Special information line:
This can allow you to add extra information to your pledge form. For example, you can inform your
employees where to drop off their pledge packages. There is a max of 36 characters.
