When completed by the IEP Team, this checklist becomes a part of the student’s IEP.

ACT Aspire
1. Student will participate in the ACT Aspire in reading and mathematics in Grades 3-8. * LEA may choose to assess
additional subjects.
2. Student will participate in the ACT Aspire in science in Grades 5 and 7.
3. Student will participate in the ACT Aspire in reading, mathematics, English, writing and science in Grade 10.
4. No accommodations are required for student to participate.
5. Accommodations are required for student to participate. (See attached ACT Aspire Accommodations Checklists.)
6. Student will participate in the Alabama Alternate Assessment in reading and mathematics in Grades 3-8. * LEA may choose
to assess additional subjects.
7. Student will participate in the Alabama Alternate Assessment in science in Grades 5 and 7.
8. Student will participate in the Alabama Alternate Assessment in reading, mathematics, English, writing and science in Grade 10.
