INSTRUCTIONS FOR COMPLETING FORM UP-8-7 FOR ESTATES

1. Name of Estate/Address of Admistrator/trix -- Complete the name and mailing address.

2. Estate Tax ID or Social Security Number: Enter the nine-digit federal tax ID number assigned to the estate.

If there is none, then enter the deceased individual's SSN. The report cannot be proccessed without this number.

3. Administrator/trix mailing address: Enter the mailing address of the appointed administrator for the estate.

4. Administrator/trix Name, phone, fax, email: Enter the name, phone number, fax number, and electronic

mail address of the person who is the Administrator/trix of the Estate.
