Financing Statement
Please type or print form information. Read all instructions. Follow instructions completely. Enclose proper fee
amount. Filings will not be recorded until receipt of the correct filing fee amount.
Complete form clearly; mistakes may result in legal consequences. If you have questions, consult an attorney.

Enter the full legal name and mailing address of the
business or individual debtor. Identify the debtor as a
business or individual by checking the PERSONAL or
BUSINESS BOX. Both boxes may be checked if the
individual debtor name and the business name are
identical.
Filings should be made under the full legal name of the
individual or business. Enter personal names on the form
as Last, First, Middle. Do not abbreviate name(s). Be sure
that the debtor name information is completely legible.
Keep in mind that a searcher may later have difficulty
finding a filing that has been submitted under an incorrect
or improper name.
Additional Trade names, DBAs, or AKAs may be included
in Box 1 for entry on the data base.

A Transmitting Utility is any person primarily engaged in
the railroad, street railway or trolley business; the electric
or electronic communication transmission business; the
transmission of goods by pipeline or the transmission or
the production and transmission of electricity, steam, gas
or water; or the provision of sewer service.
Products of Collateral are products made from security
interest perfected collateral. The collateral has lost its
original identity through processing, manufacturing,
assembling or commingling.
