DO NOT RETURN THIS FORM UNLESS YOU ARE ELECTING TO ATTEND A DRIVER IMPROVEMNT SCHOOL. IF YOU
HOLD A CDL (CLASS A, B OR C), YOU AREN’T ELIGIBLE.

It is your responsibility to contact Driving Instruction approved by the State of Florida. This form must be returned with payment within
(30) calendar days from the date you received the citation (ticket). Pursuant to Florida Statute 318.14(9), if you have received a traffic
citation for an infraction that does not require a mandatory court hearing, you may elect to complete a defensive driving school. Upon
completion of the school adjudication will be withheld and no points will be assessed against your driving record.
THE ORIGINAL
SCHOOL CERTIFICATE must be in the CLERK’S office within (90) days of the date the citation was paid. TO BE ELIGIBLE FOR THE SCHOOL
OPTION 365 days (12 months) must have passed since you last signed an affidavit to attend the Defensive Driving School.
This affidavit must be notarized and dated within 30 days of the date the citation was issued.
You must attend an approved school within the STATE OF FLORIDA.
A copy of this affidavit must be presented at the time of your class.
You may not have used this option (5) times within the past (10) years.
You must complete the Defensive Driving Course (4hrs.) and mail proof of school completion to the CLERKS’S office.
Driving schools are not responsible to provide proof of completion to the CLERK’S office.
FAILURE TO COMPLETE THE COURSE IN TIME WILL RESULT IN THE State of Florida suspending you driving
privileges.
You will be required to pay a late, processing fee and possibly a $60.00 reinstatement fee.
Points will be assessed on your driving record.
No extension will be granted.
ELECTING THE SCHOOL OPTION WHEN NOT ELIGIBLE WILL RESULT in the State of Florida suspending your driving
privileges.
The Dept. of Highway Safety & Motor Vehicles will notify if not eligible.
Points will be assessed on your driving record.
You will forfeit one of your (5) elections.
You will be required to pay a late fee, a processing fee and possibly a $60.00 license reinstatement fee.
I, hereby swear or affirm that I am eligible for the option of attending defensive driving school in the STATE OF FLORIDA ONLY; that, I
understand the requirements stated above. The selection of this option cannot be changed.
