WHERE SHOULD YOU FILE THIS FORM?

This application must be filed at any of the Taxpayer’s Service Centers located in Aguadilla, Arecibo, Bayamón, Caguas,

Mayagüez, Ponce, San Juan (Intendente Ramírez Building) and Hato Rey (Mercantil Plaza Building). For the addresses of the

NOTIFICATION REGARDING THE IVU LOTO OVERSIGHT PROGRAM

Sections 4030.01(a)(3), 6054.01(a)(2)(C) and 6054.01(a)(4) of the Internal Revenue Code for a New Puerto Rico authorize the

Secretary of the Treasury to require every merchant the installation of a fiscal terminal, or the modification of an existing application,

in each one of the points of sale located in your commercial locations with the purpose of allowing the IVU Loto oversight

program.

Once you receive your Merchant’s Registration Certificate, you are required to register for purposes of the IVU Loto

oversight program. This registration must be made within the next 30 days after the issue date of the Merchant’s Registration

Certificate, through the

website or by calling (787) 200-7900 Option Number 4. The registration for

purposes of IVU Loto requires the completion of a questionnaire of only nine (9) key questions.

Once you complete the questionnaire you will be notified if you are eligible or not to install a fiscal terminal, or modify the existing

applications, in each one on the points of sale of your commercial location.
