**This form MUST be used to change the registered agent and/or registered office of a Limited Partnership
(Idaho Code section 53-204) or a Limited Liability Partnership (Idaho Code section 53-3-1001B).
**This form MAY be used to change the registered agent and/or registered office of a Corporation
or Limited Liability Company.
**The form must be executed by:
Corporation: Chairman of the Board, President, Secretary, or another officer
LLC: Manager of the company, or if no managers, by its members
LP: General Partner
LLP: Partner
**The newly appointed registered agent must sign the change of registered agent accepting such position.
**A registered agent may sign and file a statement of change only to the address of the
registered office without a signature from any officer, manager, partner, etc.
