required Real Property Income and Expense Statement (RPIE-98) because I have already submitted the most up-to-
date information to the Tax Commission. Along with this Agreement form I have included an addendum or computer-
ized rent roll if my property is completely commercial or mixed-use with commercial portions and has a final actual
assessment of more than $1,000,000.
I certify that all information in this document is true and correct to the best of my knowledge and belief. I understand
that the willful making of any false statements of material fact herein will subject me to the provisions of law relevant to
the making and filing of false instruments and will render this filing null and void.
