due on fuels removed from a terminal other than by bulk transfer. By signing below, the Shipper

whether Distributor, Supplier, Permissive Supplier, or any other entity, accepts the responsibility to
precollect and remit the tax due. The amount precollected will be passed to the Supplier for
remittance to the State of New Jersey.
The following Bills of lading are covered by this notice. If this is a Blanket Notice, indicate that by
writing "ALL FOR THIS PERIOD" in the first space. A Blanket Notice is the Shipper's declaration of
responsibility to precollect and remit all tax due for all products shipped with this Transporter
during this period.
