Principal, together with official copies of the student’s academic records. We ask that you complete the following information and submit this form
to the school.

complete this form, keep the original and send a photocopy directly to the requesting school. Your comments will be held in the strictest confidence.
PLEASE ATTACH THE OFFICIAL TRANSCRIPT FOR THE STUDENT AS PER ABOVE AUTHORIZATION.
If you do not feel that you are the appropriate person to fill out the recommendation form, please pass it on to the division head, guidance
counselor, or student’s advisor. Similar forms will be sent to the student’s teacher(s).
