Paperwork Reduction Act

The use of this new form is considered an information

collection that, ordinarily, would be subject to review and

clearance under the Paperwork Reduction Act procedures.

However, section 503(c) of the Intercountry Adoption Act

(IAA), Public Law 106-279, waives the requirement of the

Paperwork Reduction Act with respect to information

collected for use as a Hague Convention record. Accordingly,

this form does not require an OMB Control Number.

The public reporting burden for this collection of information

is estimated at 30 minutes per response, including the time for

reviewing instructions and completing and submitting the
