The Victim Impact Statement is different from the statement about the actual events of the criminal
incident that you may have provided law enforcement earlier. This is your chance to tell the prosecutor,
judge and defendant how the crime made you feel and the impact it has had on your life since happening,
as well as telling them what sentence you would like to see imposed on the defendant. This can be a very
important and useful tool in telling YOUR story; however, you are under no obligation to fill it out. We
ask that if you choose to complete it, please notarize the Victim Impact Statement. You will want it
available to our office and/or judge as soon as possible so it can be read at any hearing that the defendant
could enter a guilty plea or be found guilty. Unless otherwise requested by you, the statement can be
provided to the defendant’s attorney within discovery requests.
The following suggestions and the attached forms are offered only as a guide. Please answer as many
questions as you wish. If you need more space or you wish to provide information in a different way,
please use as much paper as you need, and attach the narratives to this form when you return it. If you feel
uncomfortable in any way using this form, you may write a letter to the judge and tell him how this crime
has affected you and those close to you.
