be made under section 150(11) Companies Act 2014. The exclusion cannot be provided
under any other circumstances.
A new application can only be granted where the form T1 is accompanied by a supporting

statement issued from an officer of the Garda Síochána not below the rank of Chief
Superintendent. The exemption can only be made in respect of notifications of new addresses.
It is not possible to remove any residential addresses that are placed on the public record.
Where Form T1 is accompanying an application for appointment, please state which submission

Form A1, application to incorporate an Irish company
Form B10, appointment of director/secretary
Form B10a, notice of change in director's name/address - multiple companies
Q1 form, application to register migrating company by means of continuation

When you have completed and signed the form, please file with the CRO. Please send with the
CRO address
other documents, (if applicable), that the form T1 references together with the prescribed fee to
the Registrar of Companies at:
Companies Registration Office, Bloom House, Gloucester Place Lower, Dublin 1.
All such applications must be marked "For the attention of the Registrar"
Payment
If paying by cheque, postal order or bank draft, please make the fee payable to the Companies
Registration Office. Cheques or bank drafts must be drawn on a bank in the Republic of Ireland.
