Thank you for requesting information about the CC/LDI integrated pharmacy services prescription Discount Card

Program.

Enclosed is a packet of information about the program including an application for enrollment. When

you decide to enroll, please fill out the Enrollment Application and your annual enrollment fee of $15.00

(per person). Please make check or money order payable to LDI integrated pharmacy services and mail

along with the enrollment form to:

Upon receipt of your enrollment form and fee, you will receive a prescription identification card from

LDI integrated pharmacy services. Present this card to your pharmacist to receive discounts on your

prescriptions. Those prescriptions classified as acute/emergency and maintenance can be filled at most

retail pharmacies such as Walgreen’s, Wal-Mart, Schnucks, and other participating retail pharmacies.

Acute/emergency medications are those prescribed for a short period of time such as antibiotics,

seasonal allergies, etc. Maintenance medications are those taken on a regular basis for chronic

conditions such as heart, high blood pressure, diabetes, etc.

Should you decide to use the LDI mail order pharmacy service, we will mail your prescriptions

directly to your home. There is a delivery service charge.

All medications are mailed in plain unmarked packages for precaution and safety.

If you prefer, you can also bring your prescriptions into LDI's pharmacy and have your

prescriptions filled while you wait. Either way, you will receive the same discount and service as you do

with the mail order service.

If you are undecided about using the LDI mail order program, please use the attached CC/LDI

Prescription Price Form and tell us the name, strength, quantity and days supply of the medications you

are currently taking. LDI's pharmacy will fill in LDI’s price and return to you for you to

compare. This will assist you in deciding whether to take advantage of LDI’s mail-order program or to

stay with your current pharmacy.

If your doctor prescribes a maintenance medication for a chronic condition and you wish to have it filled

immediately, you can take the prescription(s) directly to any participating retail pharmacy for filling.

When you are ready to refill the prescription, simply call LDI's pharmacy, give them the

information on the prescription label, and they will take care of having the prescription transferred to LDI's

pharmacy and sent out by mail directly to you.
