– This checklist is for applicants using DOH Form DH-1054 to apply for or renew a license for the following use categories:
1A – 1C, 2A – 2C, 3A, 3B, 3F(1) – 3F(3), 3G, 3H, 3I(1), 3I(2), 3J, 3K, 3L(2), 3L(3), 3L(5), 3M(1), 3N, 3O, 3P, or 6. Refer

Enclose relevant documentation on the rad. safety training/experience for the RSO &
