This application form is not for sale. If someone sold this copy to you, take note of his/her name and make a report to the
Commanding General, Philippine Army.

Before filling-up, you can reproduce the form for friends who might be interested in joining the Philippine Army either as
Officers, Enlisted Personnel or Civilian Employees.
3.
Verify all entries to make sure that they correspond to official documents. Any false entry may result to disqualification or
future dismissal.

Accomplished form may be sent directly through mail to the address indicated below or course it through the nearest
processing center
