ABANDONMENT
Pursuant to HRS §436B-9 your application shall be considered abandoned and shall be destroyed if you fail to provide evidence
of continued efforts to complete the licensing process for tw o consecutive years. The failure to provide evidence of continued
efforts includes but is not limited to: (1) failure to submit any required information and documents requested by the licensing
authority w ithin tw o consecutive years from the last date the documents and information w ere requested, or (2) failure to
complete any additional requirements for licensure that remain after approval of your application, such as attempting to complete
an exam requirement, w ithin tw o consecutive years from the date your application w as approved, or (3) failure t o provide the
licensing authority w ith any w ritten communication during tw o consecutive years indicating that you are attempting to complete
the licensing process. If an application is deemed abandoned the applicant shall be required to reapply for licensure and comply
w ith the licensing requirements in effect at the time of the reapplication.

By signing your application form (VET-01), you are cert ifying that you have read and w ill abide by the provisions of Haw aii
Revised Statutes Chapter 471 (the Board's law s), Haw aii Administrative Rules Title 16, Chapter 101 (the Board's rules) and
Haw aii Revised Statutes Chapter 436B (the Professional and Vocational Licensing Act).

LICENSURE & BIENNIAL RENEWAL
After all requirements are met, license fees w ill be due. You w ill be sent notification of amounts of these fees at the appropriate
time.
All veterinary examiners licenses (regardless of issuance date) expire on June 30 of each EVEN-NUMBERED year and must be
renew ed on or before the license expiration date. Renew als that are received after the license expiration date are subject to late
renew al fees.
A forfeited (non-renew ed) veterinary examiners license can be restored up to 3 years from the date of forfeiture, upon the filing
of a restoration application and payment of all required fees.
After three (3) years from the date of license forfeiture, a forfeited license may be restored upon the filing of a restoration
application, payment of all required fees, and verification provided to the Board that the former licensee is qualified to practice
veterinary medicine.
CHANGE OF ADDRESS
You must immediately report any changes of address to the Licensing Branch in w riting, so that your records can be updated.
