Total gallons from original schedule

List only the information for the transactions to be added or subtracted from the schedule. Gallons being subtracted

should be noted by ( ). Attach a copy of the bill of lading for all gallons being subtracted from the schedule.

Provide a detailed explanation for each transaction being added or subtracted from the schedule.

Calculate and enter the net amended gallons for the schedule.
