Who Can File this
Any organization that is considered to be tax-exempt under section 501(c) (3)
Form:
of the Internal Revenue Code to which a vehicle has been donated.
Please Note:
When a vehicle is donated, for the purpose of being transferred to another
person, and is titled in the name of a non-profit organization, it is subject to
$18.00 title fee, Title Ad Valorem Tax (TAVT) in an amount equal to one
percent (1%) of the fair market value of the motor vehicle and any applicable
penalties.

This form must be submitted at time of processing the title transfer.

Under penalty of perjury, the undersigned recipient affirms that the vehicle identified in the
application attached was transferred to said entity as a gift by the donor (prior owner) for the purpose
of being transferred to another person and that no payment was given or received in consideration of
the transfer of ownership.

correct to the best of my knowledge.
