Kansas law requires the applicant for any logo royalty fee license plate, paying the
fee to the county treasurer, to consent to allow the Division of Vehicles to provide
owner/lessee and vehicle information to the sponsoring organization of the logo
royalty fee license plate. By affixing your signature to this form you are providing
this consent for the initial issuance, future transfer of the plate to another vehicle
as well as any replacement plate issued by the division.
By paying the logo
royalty fee along with the registration renewal fee and property tax for all future
registration renewals of the plate listed herein or replacement for same, you are
maintaining this written consent in an active status.
