INSTRUCTIONS:
a)
The form is to be filled by the Subscribers whose PRAN is in freezed status and needs to be un-freezed.
b)
The form is to be submitted to the associated POP-SP.
c)
The Subscriber shall provide copy of PRAN card along with the application form.
d)
Applications incomplete in any respect are liable to be rejected.
e)
Subscribers are advised to retain the acknowledgement slip signed/ stamped by the POP-SP where they submit the application.
f)
The Penalty of Rs. 100 (per year of freezing) is applicable to unfreeze Tier 1/Tier 2 accounts. For Swavalamban Subscribers the penalty amount of Rs.
25(per year of freezing) is applicable.
g)
The contribution amount required to unfreeze the account is mentioned below
Minimum Tier-1 unfreeze amount = Rs 6000-amount already contributed in the financial year) calculated for each year + Rs. 500 for current
financial year + penalty of Rs. 100/- for each year of freeze. If the contribution amount less than the minimum expected amount is uploaded
than the CRA system will reject the contribution during the upload it self.
Minimum Tier 2 unfreeze amount= Difference between the amount lying in the account as on the date of freezing and Rs.2000/- + Rs.250/-
(minimum contribution) + Rs. 100/- as penalty.
h)
The POP before accepting this form should verify that the contribution accepted for unfreezing should be greater than minimum amount as prescribed by
the PFRDA.
i)
POP charges of as define by PFRDA for the contribution transaction will be applicable.
j)
Request for Tier II re-activation (Option 2) can be submitted only if Tier I is active.
k)
CRA Service charges as defined by PFRDA will be applicable for the contribution transaction.
