For any of the documents you need to send off for, just have them send the certified copies to Human
Resources (H.R.) for you. Here’s the address: City of SV, Attn: H.R., 1011 N. Coronado Dr., SV
85635. If they send them to you, DO NOT open the envelopes. Just turn them in or send them in,
sealed, to H.R. If the envelope(s) are opened, they will be considered invalid.
Birth Certificate Requirement:
• If you have your ORIGINAL birth certificate, there are 2 options:
1) If you are turning in your birth certificate, in person, at Human Resources, they can make a
photocopy of it right then and there, indicating it is a true copy. That will suffice.
2) If you are mailing in your birth certificate, you can take your original to a Notary Public. Ask
them to photocopy the original, notarize the copy, stating it is a true copy. If a Notary Public
cannot or will not assist you, you will need to order a certified copy through the Bureau of
Vital Statistics. You can do that online if you wish. It’s easiest to “google” the Bureau of Vital
Statistics and go to the state you were born in.

H.S. & College Transcripts requirement:
• Human Resources will accept photocopies to start, but you’ll have to order certified copies from
each of the schools you have attended. We recommend ordering these immediately.
• Feel free to include info about any awards, honors, training you have received. Those are “feathers
in your cap!”
Questionnaire:
• Make sure to get the “Authorization for Release of Information” sheet notarized. If you are a
local resident, there are Notary Publics in City Hall you can use the day you turn in the
questionnaire, if you’d like. If this page isn’t notarized, your background investigation will not
proceed until it is.
• There’s a section at the end for anyone with prior law enforcement experience. You can skip these
questions if you have none.
• There is a section requesting you to list some neighbors. Do not leave that section blank. We
need three or four. They don’t have to be current neighbors, but they shouldn’t date back further
than two years or so. You may have to go introduce yourself to some neighbors if you can’t
provide three or four. The Police Department will not share this information with anyone.
• Make sure to be neat, honest and thorough when filling this out. Everyone makes mistakes!
Deception and lying is not tolerated. That will flush an applicant quicker than anything. Do not
submit incomplete information or the hiring process will be delayed.
