REQUIREMENTS FOR COMMERCIAL DRIVERS
IMPORTANT: New Federal Regulations require submission of additional proofs which prohibit you
from renewing your Commercial Driver License (CDL) by MAIL. See the red text
below for additional information.
Read page 2 of this form to find out about the Medical Certificate requirements and how they
affect your CDL renewal.
The U.S. Department of Transportation (USDOT) Federal Motor Carrier Safety Administration regulations
establish minimum qualification standards for persons who operate (or expect to operate) commercial motor
vehicles, and establish requirements for state-issued commercial driver license documentation. The regulations
are found at 49 CFR 383.
FOR ALL RENEWALS ON OR AFTER JUNE 18, 2015:
US Citizens and Lawful Permanent Residents:
- You must visit a DMV office and provide proof of your citizenship or unrestricted Lawful Permanent
Residency.
- One proof of residence is required each time you renew. If the address on your license renewal invitation
(MV-2) is correct, your MV-2 will serve as acceptable proof. Otherwise, you may present any of the
acceptable residency proofs listed on form ID-44CDL.
Applicants from a foreign jurisdiction:
- You must visit a DMV office and provide proof of your legal presence in the United States.
- One proof of residence is required each time you renew. If the address on your license renewal invitation
(MV-2) is correct, your MV-2 will serve as acceptable proof. Otherwise, you may present any of the
acceptable residency proofs listed on form ID-44CDL.
Note: Original documents must be provided at your DMV office.

Citizenship, Lawful Permanent Residency, foreign legal presence and New York residency.
