CHECKLIST FOR FILING AN APPEAL WITH THE
MINNESOTA COURT OF APPEALS
Below is a general checklist for your assistance in filing an appeal. It is wise to consult the court
rules or any applicable statutes for information on timelines and procedures, etc., regarding the
type of appeal you wish to file.
You can find the court rules regarding civil, criminal, civil appellate and juvenile procedures, as
well as other rules and information, on the Internet at

1. Original Notice of Appeal
(Form
103A) and one copy. See
Rule 103
under rules of
civil appellate procedure-for additional information, and Rule 28 under rules of criminal
procedure for criminal type of appeals.
2. Original statement of the case
(Form
133) and one copy. See
Rule 133.03
under rules
of civil appellate procedure for additional information.
3. Copy of decision (order or judgment) appealed. This document may be obtained from
the
district
court.
4. $550.00 filing fee or proof as to why no filing fee is required, such as an order from the
district court granting In Forma Pauperis status on appeal. See
Rule 109
under rules of
civil appellate procedure for more information.

5. Certificate of service for the appeal papers on opposing party(ies) and proof of service
for the notice of appeal on the district court administrator. Forms:
