Parking Application Form

Instructions

On the second and third pages of this document you will find the forms that need to be filled out to apply for a

parking space with us. The information form must be notarized and filled out by the REGISTERED OWNER

of the vehicle.

Please make sure you include a copy of the insurance card, the driver’s license of the person who will use the

spot on campus, the vehicle registration and your non-refundable payment. Parking for one (1) semester is

$475, parking for the entire year, paid in advance, $900. Checks should be made out to: St. Thomas Aquinas

Chapel.

When you arrive on campus please stop by the St. Thomas Student Center to pick up your parking pass. This

must be displayed on the dashboard at all times.
