RML Analysis Request Form Instructions
When completing this Analysis Request Form please do the following:
Enter your sample ID and ensure that it matches the ID label on your vial.
Include your name, company name and address, and email address; it is essential that we have this information for correct re-
porting of your results.
Include a phone number for contact in the event the analyst has specific questions that may be best answered via an in-depth
phone conversation rather than a brief email.
Enter the elements present in your sample (All Elements Present) so that our analyst can appropriately prep the sample and ac-
count for any interferences in the analysis. Alternatively, you can provide the empirical formula if known.
Include either a PO# or credit card information; we do not process samples for analysis without payment information.
Choose your required service level (be aware of the additional charges for Rush and Priority Rush service). Please note that Rush
and Priority Rush services MUST be scheduled and confirmed by us to ensure your spot in the analyses queue for that turnaround.
The “Analyses Requested” section must include ALL testing you are requesting for the sample(s) (example: CHN; Pb by ICP-MS; KF;
Na, Ca, MG by ICP-OES). We do not perform tests that are not requested in writing.
Immediately below “Analyses Requested” provide the expected range or value for C, H, N, KF; if you are requesting any other ele-
ment (for example S, Na, Ru) enter the element symbol, the expected range or value, and choose the appropriate unit of measure.
If you have additional information to add about your sample(s) or their handling, provide that in the “Remarks” section.
Choose any additional services required from the checkbox list; please note that default options are automatically checked on the
form.
Use Page 2 for additional samples being submitted in the same manner as Page 1.
This completed form MUST be included in the box with your samples; we do not accept forms submitted electronically.
Please contact us with any questions you may have when completing the form. Delays in sample analysis may occur with incom-
plete or incorrect information provided.

If repeat analyses are required due to lack of information, extra charges may apply.
