Mandatory Graduate Information and Ticket

Request Form

Parents/Guardians of the Class of 2015,

This letter introduces information needed for graduating students and the method of collection. The

following form will go LIVE on the Wootton website beginning Thursday, October 16, 2014, and will

be available through Thursday, November 6, 2014. We are providing a copy of the online form so

families may begin collecting information. This information must be entered for each graduating

senior. Detailed instructions will be delivered to families via automated call and will be posted on the

Wootton website prior to October 16.

There are two parts to this form:

Part one collects information for your child’s diploma, cap/gown order, and pronunciation of his/her

name for the graduation ceremony.

Part two contains information about the number of tickets available to each family, extra ticket

requests, and special seating requests. Extra tickets will be distributed equitably on a first come, first

served basis, based upon the date/time the Graduate Information Form is submitted. Historically, we

have been able to provide at least one extra ticket for families in need, however, this depends on the

number of students graduating, and the number of families requesting extra tickets. The distribution

list will be generated based on the date/time the Graduate Information form is submitted. There is

no guarantee that all extra ticket requests will be granted. We will accommodate as many

requests as possible in a fair and equitable manner.

Graduate Information and Ticket Request Form

General Information for Graduates and Parents/Guardians:

Graduation will be held at DAR Constitution Hall on Wednesday, May 27, 2015 at 2:30pm. Mandatory

graduation rehearsal will take place at Wootton HS’s main gym on Tuesday, May 26, 2015 at 9:00am. Caps

and gowns will be distributed at the end of graduation rehearsal. All obligations must be cleared to receive

the cap and gown.

Tickets will be distributed at the end of each of the mandatory parent/guardian graduation meetings.

Parents/guardians are required to attend one of the three meetings. The dates for the meetings are as

follows:

Senior fees are due by Monday, January 5, 2015 and can be paid through the Wootton High School Online

Store.

Graduation announcements are offered through Balfour/Jostens (TBD). Information can be found on (this
