Instructions:
1. Use the following document for application to the Bishop for consent for remarriage.
2. The priest should include a cover letter with pertinent details.
3. The priest completes the first two-page section.
4. Marriage applicant(s) complete(s) the separate one-page section.
5. A copy of the divorce decree(s) must be included.
6. A brief account of the marriage history (Item 10, page 3) must be included.
7. A MINIMUM OF 30 DAYS IS REQUIRED FOR SUBMISSION TO THE BISHOP OF THIS
INFORMATION FOR APPROVAL.
