Scoring Instructions

In assessing Follow-Up Vanderbilt Assessment Scales, the two important components to track are SYMPTOMS

AND IMPAIRMENT in both settings (i.e. Home and School).

Questions # 1 – 9 screen for Inattentive Symptoms

Questions # 10 – 18 screen for Hyperactive Symptoms

Questions A - H screen for Impairment

GOAL: >25% Reduction in follow-up Total Symptom Score AND normalization in at least ONE area of

Impairment

OPTIMAL GOAL: >50% Reduction in follow-up Total Symptom Score AND normalization in ALL areas of

Impairment

Inattentive Symptoms

1. Count the number of positives (2s and 3s) for #1-9

2. Count the number of positives (4s and 5s) for #19 -26

Still meets criteria for significance if:

6 or more Positives (2s and 3s) for #1-9

and

1 or more Positives (4s and 5s) for A - H

Hyperactive Symptoms

1. Count the number of positives (2s and 3s) for #10-18

2. Count the number of positives (4s and 5s) for #19-26

Still meets criteria for significance if:

6 or more Positives (2s and 3s) for #10-18

and

1 or more Positives (4s and 5s) for A - H

To Calculate the Total Symptom Score (TSS)

: Find the sum of all of the 0s, 1s, 2s, and 3s that have been

circled in Questions 1 – 18. The maximum score in this section is 54.

To Calculate the Average Performance Score (APS)

: Find the sum of all the 1s, 2s, 3s, 4s and 5s that

have been circled in Questions A through H. Next, divide this calculated sum by the total number of questions that

have been answered.
