PRE-NUPTIAL INVESTIGATION
1. This form is ordinarily to be completed by the priest/parish minister who prepares the
couple for marriage and filed in the parish where the marriage takes place. In case a
dispensation from the canonical form of marriage is obtained, the pre-nuptial file and
record of the marriage are kept in the parish of the Catholic party where the
investigation was made. (Canon 1121.3)
2. This form must be sent to the Chancery when a dispensation or permission is
requested from the Local Ordinary (the Diocesan Bishop, Vicar General or their
delegate for matrimonial dispensations)
3. Baptismal certificate(s) issued in the past six months must accompany this form.
