This form is for prospective students of Carolinas’ Music and Arts Conservatory Program. Before it can be accepted by the
Divisional Music Department, it must be completed and signed by each student, parent/guardian, and the corps officer.
Acceptance to the program is not granted on a first-come, first serve basis. The following requirements must be met:
1.
students must be 10 years old by June 18, 2012
2.
students must have an ongoing-involvement in music or artistic ministries at the corps level;
3.
students/parents commit the student to remaining in the program for its complete duration;
4.
students must bring their instrument with them to camp (except piano/percussion)– the music department is not responsible for providing
instruments for students;
5.
each student, the parent/guardian, and the corps officer agrees to each of these terms included by signing this application.

Describe any musical artistic experience that you may have. You may
include any Salvation Army programs (including other regional, divisional, or territorial events) as well as other programs including
school or community programs. Students are selected based on participation at the corps level (where available), so it is
imperative that you complete this information.
Camper Agreement:
If I am accepted as a student for The Salvation Army Carolinas’ Music Conservatory I will:
Maintain Christian standards in my behavior, dress, relationships and language;

Bring every thing with me that I will need while at camp (including uniform, instrument, etc - list to be mailed later);

Obey the rules that are set for me at camp;

Respect all staff members and my other campers;
•
Work diligently in order to do my best;
•
Attend all meetings and participate in all that is required of me at camp;
