Use this form when submitting a Business Resolution or Amendment to Rule by petition. Refer to the
Guide to Business and to the UUA Bylaws Article IV for further information on how to write resolutions and
amendments. Proposed Business Resolutions and Amendments to Rules appear on the Tentative Agenda.
In order for a Business Resolution to appear on the Final Agenda, it must receive a majority of votes in the
Congregational Poll.
Required Materials (must be submitted together):
• Exact text of resolution or amendment
• Certification forms (below) signed by not fewer than 250 members of UUA certified (in 2016) congregations,
with no more than 10 members from one congregation.
