Complete the following steps for each person attending:

1. Go to

a. Select the Type of Visa as Single.

b. Enter information in Step 1.

c. Enter information in Step 2 (can enter same person twice)

d. Enter Passport & Visitor Contact Info in Step 3.

i. Place of Issue: United States

ii. Issuing Authority: US DOS

iii. Proposed date of entry: 27/12/2014

iv. Proposed date of departure: 05/01/2015

1. Places to be visited: Greented International School, PO Box Private Bag,

Nakuru, Kenya, Phone: 25450770

e. Answer YES to question in Step 4.

Print resulting visa application form in Step 5. Suggest you save it as a PDF.

2. Complete questions 10 & 11 after printing, and then sign.

3. Verify your passport has:

a. At least 6 months validity from the date the group returns. If your passport expires on or

before July 6, 2015, order a new one.

b. Your signature on it.

c. At least two blank pages available.

i. NOTE: Include Passport in packet to Springfield Travel.

4. Obtain two (2) original passport-sized pictures with:

a. Your full face, front view and open eyes, no glasses

b. Dimensions of 2” x 2”

c. A plain white or off-white background

NOTE: No copies, instant, or scanned photos accepted

5. Include a $80 check or money order made payable to Springfield Travel (this covers fee for the

Consulate of Kenya and processing)

6. Complete information on packet checklist (last page of instructions) and include on top of packet.

7. Fedex Overnight all of the above items to Springfield Travel Services, Swamp Corps Kenya,

ATTN: Malinda Ford, 1409B West Sunshine Street, Springfield, MO 65807.
