Illinois Residency Verification
2016-2017
I understand that I must be a resident of Illinois to receive financial assistance under the Illinois
Student Assistance Commission (ISAC) Monetary Award Program. Further, I understand that
Illinois residency is defined by the ISAC to be the following:
"An Independent Student is a Resident of Illinois if the applicant has a true, fixed and
permanent home within the State of Illinois (at the time of application) and has so resided
for a period of 12 continuous, full months immediately prior to the start of the academic
year for which assistance is requested."
I certify that I was living in the State of Illinois at the time I filed the 2016-2017 Free
Application for Federal Student Aid (FAFSA) and that I have physically resided in the State of
Illinois 12 continuous full months since
August 24,

Utility or rent bills in my name
Illinois High School or College transcript
Illinois Auto Registration Card

State of Illinois Identification Card issued by the Secretary of State
Statement of benefits history from the Illinois Department of Public Aid
Statement of benefits history from the Illinois Department of Employment Security
