Illinois Residency Verification
2016-2017
I understand that I must be a resident of Illinois to receive financial assistance under the Illinois
Student Assistance Commission (ISAC) Monetary Award Program. Further, I understand that
Illinois residency is defined by the ISAC to be the following:
"An Independent Student is a Resident of Illinois if the applicant has a true, fixed and
permanent home within the State of Illinois (at the time of application) and has so resided
for a period of 12 continuous, full months immediately prior to the start of the academic
year for which assistance is requested."
I certify that I was living in the State of Illinois at the time I filed the 2016-2017 Free
Application for Federal Student Aid (FAFSA) and that I have physically resided in the State of
Illinois 12 continuous full months since
August 24,
2015. I have attached the following
document(s) with an issue date prior to
August 24, 2015
to verify my Illinois residency in
accordance with the above definition:
 Illinois Driver's License
 Utility or rent bills in my name
 Illinois High School or College transcript
 Illinois Auto Registration Card
 Residential lease with my name appearing on the lease
 Wage and tax statements (IRS W2 Form)
 State of Illinois Identification Card issued by the Secretary of State
 Statement of benefits history from the Illinois Department of Public Aid
 Statement of benefits history from the Illinois Department of Employment Security
