Please read and initial the following statements:
I agree to follow all local and state ordinances pertaining to this pet.
I agree to return this cat to the VHS if I am unable to keep it.
I agree to provide ID tags for this pet at all times.
I agree to notify the VHS for assistance if this pet is lost or stolen
I understand that the VHS adopts, not sells, pets. Therefore, just as with a child we feel that this is a serious
life-long commitment. Pets adopted from the VHS are adopted for the life of the pet.
I agree to take my pet to the vet to discuss necessary vaccinations, preventative treatments and a physical exam.
I understand that upper respiratory conditions are very common in animal shelters. The VHS is no exception.
Upper respiratory infections are usually no more than a cold. Symptoms may include nasal discharge, watery
eyes and sneezing. URI is easily treated. Please be aware that many times if a cat is not showing symptoms of
this illness while at the VHS, it may still be ill after entering a new home. For more information please speak
to a staff member.
Cats at the VHS are tested for FELV (Feline Leukemia virus), and FIV (feline immunodeficiency virus) if they
are in the “high risk group”. This includes male cats that were unaltered at the time of surrender and all cats
that have been outside. If the cat you are adopting has not been tested, you should consult with a veterinarian.
Testing does not protect a cat from these diseases.
