HEARTWORK COLORING CONTEST

OFFICIAL RULES 2016

• Contest open to elementary students in Tuolumne and Calaveras Counties (ages 12 and under).

• Each child may submit (1) one coloring sheet to the 2016 HeartWork contest.

• Color on the front of this sheet, and return completed HeartWork by Tuesday, February 16, 2016

• HeartWork can be dropped off at the front desk at the main hospital or sent via U.S. Mail to:

• Include participant’s name and age, along with a parent or guardian phone number so that selected winners

can be noti ed.

CONTEST DATES

February 9-February 16, 2016

AGE CATEGORIES

• Ages 4–7

• Ages 8–12

JUDGING

• Entries will be judged at contest end, and prizes will then be awarded.

• Entries will be judged on the basis of creativity and understanding of heart health.

• Decisions are at the discretion of Sonora Regional Medical Center and are subject to change at any time.

We reserve the right to award multiple winners and prizes.

WINNERS

• Winners will be noti ed via phone number provided on the front of this sheet.

• The winner of each age group will receive a heart health activity basket, valued at over $50.

• Winning HeartWork will be displayed in the windows of SOMA Fitness Studios at 185 S Washington St.,

Sonora, California.
