hereby register my child at Kindertown Child Care Centre. I acknowledge that I have read

the parent handbook of the Centre and understand and agree to the following:

I will abide by the policies set forth in the parent handbook and by the subsequent notices

which serve as changes or adjustments to the handbook.

I understand that all registration/enrollment forms for my child, including the medical record

must be complete before I register my child for care at the centre.

I will abide by all the policies for use of the centre and hours of operation.

I understand that my child will be released only to people designated on the authorization

and consent forms signed by myself.

I understand that all medical information must be updated in accordance with guidelines

established by the Department of Health and the Day’s Nursery Act. I will also notify the

centre of any changes in my child’s health or family situation that would affect the

registration information.
