Each quarter one picture will be available at all office locations as the Stackz Coloring Page. The feature picture

will change every quarter, and new prizes will be awarded each quarter.

One entry per patient per quarter.



In order to become a participant one must be a patient of smilebuilderz .



Ages 18 and under.



Children of employees are prohibited from entering the smilebuilderz coloring contest.



Submissions must be received by Saturday, December 15, 2012.



Drop off entries at any of our offices or mail them to our main office at:

Entries will be judged on creativity and neatness of coloring the Stackz Coloring Page in addition to the

content written in the "I love smilebuilderz because..." essay question.



The winner will be notified by mail. The first name and age of the winner will be posted at both our

office locations.



If participants would like to have their entries back after the quarterly contest, please stop by our main

office for pick up.



Participants or their guardians must “Like” or “Friend” smilebuilderz Facebook page.



By submitting this you are giving smilebuilderz permission to display your artwork, first name and age in

their advertising and marketing.



In the event you are selected as this quarter’s winner, you will be invited to stop in for a photo with the

winning page to be showcased on our Facebook page.
