He/she is a NJ resident who has not attended a school in his/her resident district for the entire year
immediately prior to enrollment in the desired Choice district.
•
He/she is attending a non-public school.
•
He/she is applying for kindergarten and does not meet the requirements for Tier 1 above.
My student is applying to the School Choice Program and may be given enrollment preference because the School
Board’s choice enrollment policy gives enrollment preference to:
•
siblings of choice students currently attending the Choice district
•
resident students who live in the Choice district and move to a different district during the school year
•
students from send-receive or limited purpose regional districts
NOTE: If there are special circumstances that would have an effect on his/her status as a Tier 1 or a Tier 2
student, please explain:

Falsifying any information on this application will result in the denial of the student’s participation in the
Choice Program.
By my signature, I certify that I have satisfied the requirement that a Notice of Intent to Participate in the School
Choice Program was provided to the Resident district by November 26, 2012 and that I am applying for my
student’s admission to the Choice district for academic reasons only and not for athletic, extracurricular, or social
reasons.
