birthday. DT vaccine is required for children who are medically

exempt from the pertussis containing vaccine. Effective

Tdap

07/01/2006 a booster dose of Tdap vaccine is required for entry into

6th grade. Students in grades 6 through 9 for SY 09/10 are required

to have Tdap provided there has been at least 5 years since their last

tetanus containing vaccine.

Polio

Minimum of 3 of all IPV or all OPV doses. One dose must be on or

birthday. A fourth dose is required for if the primary series

consisted of a combination of OPV and IPV.

Hib

Required ONLY for children up to 60 months. 2 to 3 doses

depending on manufacturer. Unvaccinated children between ages

16 and 60 months are only required to have 1 dose.

Hepatitis B

3 doses. 2 dose schedule ONLY for adolescents 11 – 15 only when

Merck Brand RECOMBOVAX HB Adult Formulation is used and

documented.

grade. Parents, at their sole discretion, may elect for their child to

not receive this vaccine.

Physical

Certificate of physical exam from licensed, qualified healthcare

Examination

provider (MCH 213 F) within 12 months of entry into primary

school (K – 5). May accept out-of-state physical exam provided it

is of the same scope as that required by VA (MCH 213 F) Required

for grades K – 5.
