Lost passport applicants are required to appear in person at their local passport acceptance agent.

These agents are located at Post Offices and County Clerk's Offices across the country. Search

1) Gather all of the required documents listed on the next page

2) DS-11 must be typed and printed (handwritten application unacceptable)

3) Take all documents in person to your local Post Office or County Clerk to be "sealed"

4) All documents are sealed EXCEPT for the Email Confirmation Copy

5) Leave the Acceptance Agent and call us at (800) 573-3738 when ready to ship
