In order to be considered for an adoption today, you must: Be at least 18 years old, have current verifiable picture

identification and understand that the Amarillo SPCA reserves the right to deny the adoption of any pet for any reason.

In exchange for being allowed to adopt the above identified Pet, I agree to the following Amarillo SPCA requirements:

_____ I am adopting the Pet for myself, and others living at my address, as a companion animal, and I will not give away, sell, or

otherwise dispose of the Pet.

_____ If at any time I cannot keep the Pet, I will make arrangements with the Amarillo SPCA to return the Pet to the shelter.

_____ If the Pet is a cat, the cat will live indoors at all times.

_____ If the Pet is a dog, my yard is fenced so that it will contain the dog, and when I, or others living in my home, take the dog

out of my home or fenced yard, the dog will be on a leash or harness sufficient to control the dog. The Pet shall have

proper identification on it, including the Amarillo SPCA tag, at all times.

_____ If the Amarillo SPCA has indicated that the Pet is an “inside dog”, the dog will live inside my home; otherwise the dog may

be an inside/outside dog. There must be adequate shelter in my fenced yard. The dog must be inside if the weather is

less than 32 degrees, if there is a heat advisory, or if there are severe storm warnings.

_____ I will care for the Pet in a responsible and humane manner, which includes supplying adequate food, water, shelter,

veterinary medical care, and annual vaccinations.

_____ The Amarillo SPCA does not guarantee or make any promises about the Pet’s temperament or behavior. I release the

Amarillo SPCA from any costs, damages, or injuries to persons or property that might be caused or are caused by the Pet.

_____ I will inform the Amarillo SPCA of my current address and phone number if either changes from that stated above.

_____ A representative of the Amarillo SPCA may call me at any reasonable time to discuss the care of the Pet; and a

representative of the Amarillo SPCA may visit my home to confirm that proper food, shelter and other care is being

provided to the Pet. If, in the representative’s determination, the animal is being abused or neglected in any way or this

Adoption Contract is not being followed in any way, the Amarillo SPCA representative has the right to remove the animal

and ownership transfers back to the Amarillo SPCA. No refund of any fees or costs will be made by the Amarillo SPCA.

_____ I promise that the information provided about me in the “Adopters Information” is true and correct. I further promise that I

will meet or perform all of the requirements of this Adoption Contract, and that failure to perform the requirements will make

adoption of this Pet null and void.

By placing my initials in the blanks above before each requirement, I acknowledge that the requirement has been explained to me

and that I understand it.
