ELIGIBILITY REQUIREMENTS
DISABLED PARKING PRIVILEGES ARE VALID ONLY WHEN THE VEHICLE DISPLAYING
THE PLATES/PLACARD IS BEING DRIVEN BY OR TRANSPORTING THE APPLICANT.
PLATES
Fee --No additional fee for disabled parking plates.
The usual vehicle registration fee applies and varies from plate to plate.

Private Vehicle
Vehicle must be registered to applicant.
Applicant must have a permanent disability.
Sections A, B, and C of this application must be completed in full. Section C is not required if applicant
already holds a valid permanent placard.
Submit an Application for Registration (VSA-14).
For special or personalized plates, submit an Application for Communiplates or Special Interest (VSA-10).
Note: For Disabled Veteran plates with the International Symbol of Access, also submit a Veteran’s
Certificate of Disability form VSA-54.
Specially Equipped Vehicle Use to Transport GROUPS of Physically Disabled Persons
Vehicle must be registered to applicant.
Sections A, and B of this application must be completed in full.
Submit an Application for Registration (VSA14).
For special or personalized plates, submit an Application for Communiplates or Special Interest (VSA-10).

PLACARDS
FEE - $5
Permanent
Applicant must have a permanent disability.
Sections A, C of this application must be completed in full for original (first time) applicants.
Only Section A of this application must be completed to renew permanent disabled parking privileges.
Section C is not required if applicant already holds disabled parking license plates.
Temporary
Applicant must have a temporary disability.
Sections A, C of this application must be completed in full.
