Government Funding for a Voice Prosthesis
Assistive Devices Program (ADP)
• Put the Equipment Supply Authorization form (green copy) in a safe place.
You need to keep this form for 3 years.
• The red number at the top of the form EA#:
is your
registration number with the ADP Program.
• You get 75% of the cost of the supplies back, up to a maximum of $1600
over the next 3 years. After 3 years has passed, contact your Speech –
Language Pathologist for another form. This will be filled out so you can
apply again for another 3 years.
• Keep the receipt and paperwork for the prosthesis that you received from
the company. Write the EA # near your name at the top of the original copy.

Keep this address and copies of all invoices for your records.
Call: 1-800-267-9458 for inquiries about your ADP account.
• After you have been reimbursed for your first prosthesis, you will be
registered in the Kingston office.
• For the next prostheses or tracheostoma supplies, send the original copy of
the receipt (with the EA # written by your name) to the address above as
soon as you receive the invoice. You do not have to wait 4 weeks for your
second prosthesis
