Illinois MAP Grant Residency Verification

The Monetary Award Program (MAP) provides grants, which do not need to be repaid, to Illinois residents who

attend approved Illinois colleges and demonstrate financial need, based on the information provided on the Free

Application for Federal Student Aid (FAFSA). Students eligible for the MAP Grant will be required to verify Illinois

residency.

Independent Students

An independent student must have resided in Illinois for 12 continuous months prior to start of classes of the

academic year the student is applying for the MAP grant to be considered an Illinois resident. For the 2016-2017

academic year, the student must have been an Illinois resident continuously from August 2015 to August 2016.

Illinois must be their permanent home.

Please check which document(s) are being submitted and attach it to this form. Note: The following

documentation must be in the student’s name and must cover the full 12 months of August 22, 2015 to August 22,

2016. This may require multiple documents to be submitted.

Valid Illinois driver's license or State of Illinois identification card, issued by the Secretary of State

If valid and issued on/before August 22, 2015

If issued after August 22, 2015, you may combine with any combination of documents listed to

prove your continuous Illinois Residency for the twelve (12) month period from August 22, 2015 to

August 22, 2016.

Residential lease agreement

Utility or rent bills (i.e water, electric, gas. A cell phone bill is not acceptable)

Illinois auto registration card

Valid 2015 State of Illinois Tax Return - signed (this will only verify 2015 residency)

Illinois high school or college transcript

Statement of benefits history from the Illinois Department of Healthcare and Family Services

Statement of benefits from the Illinois Department of Employment Security

Statement of benefits from the Social Security Administration
