8. Supporting Documentation

The renewal application must include the following documents:

[] $250 Renewal Fee (for the period of April 1 to March 31)

[] A copy of a corporation profile report issued by the Ministry of Government and Consumer Services or by a

service provider, which is under contract with the Ministry of Government and Consumer Services that is dated

not more than 30 days before the application for renewal is submitted to the Registrar and that indicates the

corporation is active.

[] A copy of every certificate of the corporation that has been endorsed under the Business Corporations Act

since the corporation’s most recent application for a certificate of authorization or for renewal of its certificate of

authorization.

[] A declaration (attached) by a director of the corporation, signed not more than 15 days before the application

for renewal is submitted to the Registrar.

a director of the above corporation, am applying on behalf of the corporation for a renewal of the Certificate of

Authorization under the Regulated Health Professions Act, and declare that the information provided above is
