QSG Consultation Form
This form must be completed in advance of all consultations. If you are a graduate student, fill out
this form with your major advisor if possible. Answer all questions to the best of your ability,
indicating N/A if the question is not applicable or you don’t understand it – it’s OK if you are not
confident in your answers. Bring 6 copies of the completed form to the scheduled consultation.
