This report is used to maintain a record of student progress and to assign a term performance designation to the

Thesis/Dissertation. You must complete this form for every term in which you are registered. PLEASE TYPE OR PRINT

LEGIBLY. In addition to assigning a grade for the Thesis/Dissertation, this report is used in the awarding of teaching

assistantships and research fellowships, so thoroughness is encouraged. It is your responsibility to: 1) ensure that your

supervisor reads the form, makes appropriate comments, and assigns a performance designation for the Thesis/Dissertation

if research work has begun, and 2) return the completed form to the Program Administrator by the due date. (See Significant

Dates ).

Failure to submit a completed and signed Progress Report to the Program Administrator by the due date may result in

the of an UNS grade in the thesis/dissertation course and a negative service indicator on SAS to prevent registration

and services.
